🏁 Pick-Up: Sunday, October 11 after Sunday service
💰 Registration: $75
👨‍👩‍👧‍👦 Sibling Discount: Families registering multiple siblings will receive a $25 discount per additional sibling.

This is an overnight youth event. Students will remain with CFC YTH leaders throughout the event and will be released to an authorized parent/guardian or approved pickup person at the end of the weekend.

Please complete all sections of this registration form and provide accurate emergency and medical information.

  Please make sure CFC YTH leaders are aware of all medications.

📵 PHONES & ELECTRONICS

NO ELECTRONICS WILL BE USED DURING THE LOCK-IN.

Students may bring their phone or other personal electronics, but all electronics will be turned off and collected by CFC YTH leaders upon arrival. Devices will be kept by the leaders and returned on Sunday before students leave.

Please do not bring valuable or irreplaceable electronics.

• Alcohol, tobacco, vaping products, or drugs


• Anything inappropriate for a youth event


• Anything that could be disruptive or unsafe


• Expensive or irreplaceable personal items

Students may bring their phones or other personal electronic devices; however, no electronics will be used during the CFC YTH Lock-In. All phones and electronic devices will be turned off and collected by CFC YTH leaders upon arrival. Devices will be securely kept throughout the weekend and returned to students on Sunday before they leave.

Please do not send valuable or irreplaceable electronics with your student.

I give permission for my student to participate in the CFC YTH Lock-In from October 9 through October 11. I understand that this is an overnight youth event and that CFC YTH leaders will provide supervision throughout the event.

I give permission for my student to be transported in connection with the CFC YTH Lock-In, including transportation to and from the event, activities, meals, and other locations associated with the event. I understand that transportation may be provided by CFC YTH, Crossroads, designated adult leaders, volunteers, or other individuals authorized by CFC YTH, using church-owned, privately owned, rented, or other approved vehicles.

I understand that transportation arrangements, drivers, vehicles, and routes may change as necessary during the event. I authorize designated CFC YTH leaders to make reasonable transportation decisions in the best interest of my student.

In the event of an accident, injury, illness, or other emergency, I authorize designated CFC YTH leaders to obtain appropriate medical care for my student, including emergency transportation, if reasonably necessary and I cannot be reached.

I understand that participation in the event and transportation involves inherent risks, including the risk of injury or accident. To the extent permitted by law, I release and hold harmless CFC YTH, Crossroads, their pastors, staff, leaders, volunteers, and designated drivers from claims arising from transportation related to this event, except to the extent caused by their gross negligence, willful misconduct, or other conduct that cannot legally be released.
